Treating clinicians use this report to update WCB Nova Scotia on a worker's psychological treatment. It asks whether the DSM diagnosis has changed, restates the treatment goals and evidence-based interventions provided, rates the response from no improvement to fully resolved, and covers day-to-day function, risk of harm, work abilities, expected duration and readiness to return to or stay at work.
The treating health professional completes and signs it with their printed name and clinic; the invoice version lists psychologists, clinical counsellors and social workers as practitioner types.

Psychology Progress Form: REPORT
Upload the fillable PDF once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

WCB's psychology assessment form says a Progress Form is required at the end of every fourth session or fourth week, whichever comes first. The progress form itself must be submitted within 14 calendar days of the last visit, according to its invoice version.
The treating health professional. The invoice version lets you identify yourself as a psychologist, clinical counsellor or social worker, and the report ends with the health professional's signature, printed name and clinic name. WCB lists the form for approved providers outside Nova Scotia.
