WCB Nova Scotia Form C: Progress Report

Out-of-province providers approved by WCB Nova Scotia use Form C to report a worker's progress after the initial assessment on Form B. The one-page May 2014 report updates the diagnosis, work status, case conference plans, expected return-to-work dates and job match, and tracks each treatment goal against its time frame. A summary records the number of treatments, the current Orebro score and overall functional progress.

Who completes it

The treating health care provider completes the whole form, including practitioner and clinic details, and copies go to the worker's physician and WCB.

Submitting and payment

How to submit it
Complete, print and sign it, write the worker's claim number on every page, and fax it to WCB.
Who pays
WCB guarantees payment only for pre-authorized services, and out-of-province providers fax a separate invoice for each worker listing the claim number, dates of service and services.

Good to know

  • Form C asks whether the diagnosis has changed, whether the injury is a sprain or strain, and whether a Form E Physical Abilities Report is attached, with a reason if not.
  • If a case conference is scheduled, give the date; if not, the form asks for the reason.
  • Overall functional progress is rated improving, no change or declining, with the reassessment date, number of treatments and current and previous Orebro scores.
  • Providers in the Work-Connected Recovery network report through Lifemark rather than on these forms; WCB lists Form C for approved providers outside Nova Scotia.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What does WCB Nova Scotia Form C report?

Progress since the initial assessment: whether the worker is working at transitional duties or pre-injury work, any change in diagnosis, expected return-to-work dates, the job match summary, and progress on each goal in the collaborative treatment plan. A summary adds the reassessment date, number of treatments, current Orebro score and overall functional progress.

Who can use WCB Nova Scotia's Form C?

WCB lists Form C for providers located outside Nova Scotia who are approved to deliver services on musculoskeletal and psychological injury claims. Providers in the Work-Connected Recovery network deal with Lifemark for forms, reporting and invoicing and do not submit forms or invoices through WCB Online.

Does Form C need a Form E attached?

Form C asks whether a Form E Physical Abilities Report is attached and, if not, why. Form E is the companion report that records lifting, carrying, pushing, pulling, grip strength and standing, sitting and walking tolerances against pre-injury job demands over up to four assessment periods.
