Nova Scotia DSP Physician Report: Medical Assessment

Nova Scotia's Disability Support Program (DSP) requires a physician report (medical assessment) from every applicant, alongside a program application, a functional assessment and a financial assessment. The program serves people with intellectual developmental disabilities, long-term mental illness or physical disabilities, generally from age 19. Care Coordinators may later ask for an updated report when a participant's needs change or capacity to consent is in question.

Who completes it

An attending physician or nurse practitioner who is familiar with the applicant and their medical history completes the report. The Care Coordinator separately completes the functional assessment of support needs.

Submitting and payment

How to submit it
The report goes to the DSP Care Coordinator as part of the application. For emergency respite admissions, the assessment documentation, including a physician report, must be completed and approved within five working days of admission.
Who pays
Applicants pay any costs and fees for the medical assessment at application (Policy 6.5.2), while for existing participants a physician's fee for a report the Care Coordinator requests may be approved as a special need (Policy 7.3.12).

Good to know

  • Policy 4.7.1 makes the physician report a condition of being considered for DSP, along with consent to assessment, a functional assessment, a program application and a financial assessment.
  • Participants are reviewed every two years; after a significant change in support needs, the Care Coordinator does a full reassessment and may obtain an updated physician report.
  • When a decision about personal care, living arrangements or health needs is required and capacity seems doubtful, the Care Coordinator may request a report to determine capacity to consent.
  • New respite placements in a Community Home or ARC/RRC also need the physician report, which the Care Coordinator submits to the Casework Supervisor with the support plan.

Frequently asked questions

Who can complete the DSP physician report?

Policy 6.5.1 calls for a medical report from an attending physician or nurse practitioner who is familiar with the applicant and their medical history. The report supports the eligibility decision, which the Care Coordinator recommends to the Casework Supervisor after the functional assessment.

Who pays for the DSP medical report?

At application, the applicant is responsible for any costs and fees associated with the medical assessment. For people already in the program, when a Care Coordinator requests a report to assess their medical conditions or their capacity to consent, the physician's fee may be approved as a special need.

What disabilities does the Disability Support Program cover?

The program policy covers intellectual developmental disability with onset before age 18, long-term mental illness, and long-term physical disability that creates significant difficulties in two or more areas of daily living. Dual diagnosis and acquired brain injury may also qualify, and applicants are generally 19 or older.
