NS Gender Affirming Surgery Application (MSI)

Nova Scotia physicians, nurse practitioners and specialists use this 5-page application to get MSI pre-approval for publicly funded gender-affirming surgery, whether performed in Nova Scotia or at the GrS clinic in Montréal. The clinician verifies clinical and surgical eligibility and attaches the required letters. All funded surgery must be approved before the procedure, and review typically takes about 4 weeks.

Who completes it

The patient completes the personal information, declaration and consent pages. A Nova Scotia physician, nurse practitioner or specialist completes and signs the clinician declaration, eligibility checklists and certification, and a credentialed assessor signs the Appendix A credentials and recommendation page.

Submitting and payment

How to submit it
Return the completed form and attachments to Medical Services Insurance by fax or mail as part of the pre-approval referral package. Review typically takes about 4 weeks, or longer if information is missing.
Who pays
There is no cost to apply for MSI pre-approval.

  • Genital surgery for adults may require at least 6 months of stable hormone therapy, while hormones aren't required for chest surgery except breast augmentation.
  • Patients 16 or older can seek an exemption for chest surgery: tick Exemption requested and attach a letter explaining the rationale, which MSI sends to the Department for approval.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can sign the Nova Scotia Gender Affirming Surgery Application?

A Nova Scotia physician, nurse practitioner or specialist signs the clinician declaration and certification. The psychosocial assessment letter must come from a health care professional with the credentials listed in Appendix A, such as the ability to assess capacity to consent and WPATH-informed training, and it can be the same person who signs the application.

Does MSI fund gender-affirming surgery outside Canada?

No. The application lists no public funding for gender-affirming surgery outside Canada, for related procedures such as facial feminization, tracheal shave or voice surgery, for surgery outside Nova Scotia without MSI pre-approval, or for take-home medications, equipment and meals. Funded out-of-province surgery is done at the pre-approved GrS clinic in Montréal.

What happens after MSI approves the request?

MSI's medical consultant issues a written pre-approval naming the approved services, the facility and an expiry date, sent to the patient, the referring and receiving providers and the Department of Health and Wellness. Adding services or extending the expiry needs a new application, and the receiving surgeon still decides whether and when to operate.
