The PPI, PPP and PRTW reports for physicians, plus initial and progress/discharge reports for chiropractors, physiotherapists, occupational therapists and psychologists.
WCB Saskatchewan is the province's workers' compensation board. Physicians complete the Physician's Initial Report (PPI) for every work-related injury, including injuries with no time lost, because it confirms disability for wage-loss benefits. The Physician's Progress/Discharge Report (PPP) follows when the condition, diagnosis or treatment changes, and the Practitioner's Return-to-Work (PRTW) Report confirms an agreed return-to-work plan. Chiropractors, physiotherapists, occupational therapists and psychologists each have their own initial and progress/discharge reports.
Send the PPI within three days of the assessment. Send the PPP about every three weeks while the worker is off work, and within three days of discharge. Both can go through the online submission form in a WCB online account or by fax, and WCB Saskatchewan pays an extra fee for online submission. Vero Documents fills fillable PDFs, such as the PPI and PPP, from the visit note and the chart, and you review and sign.

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Within three days of your assessment. The PPI is needed for every work-related injury, including no-time-loss injuries, because it confirms disability for wage-loss benefits. Submit it through the online form in your WCB online account or by fax. WCB Saskatchewan pays one report fee per day, plus an extra fee when the report is submitted online. Forward copies of special reports, such as diagnostic or operative reports.
The Practitioner's Return-to-Work (PRTW) Report confirms an agreed return-to-work plan, with hours and restrictions. The treating practitioner completes the practitioner sections, and the worker and employer sign to verify it. Copies go to the primary care provider, the employer and the worker. Return-to-work counselling, and completing an employer's own restrictions or functional abilities form, are billed under a separate fee code.
Each discipline has an initial report and a progress/discharge report: CHI and CHP for chiropractors, PTI and PTP for physiotherapists, OTI and OTP for occupational therapists, and PSYI and PSYP for psychologists. Chiropractors send the CHI within three days of starting treatment, a CHP after each block of six treatments, and a discharge CHP within seven days. Other accredited practitioners use the generic TXI, TXP and TXD reports.
MCARE is the form code on several WCB Saskatchewan authorization and funding requests. Massage therapists use one to request authorization for primary-level treatment, as do mental health providers and occupational therapists. Hearing service providers use one to request funding for hearing services or aids. Programs that take a worker into secondary or tertiary treatment send a separate Notification of Intake.
