Saskatchewan physical therapists report their first assessment of an injured worker to the WCB on this two-page form, which can also be completed online. It records the diagnosis, mechanism of injury, quantified subjective and objective findings, a functional outcome measure, a 0 to 10 recovery score and an intensity score. Management, treatment plan, employer contact, time off work and current restrictions complete the report.
The treating physical therapist completes and signs the whole report with the clinic and provider numbers. WCB Saskatchewan's standards of care expect every care provider to start communication about return to work, so item 16 asks when you contacted the employer and whom.

Physiotherapist's Initial Report (PTI)
If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Tick 'Recurrent treatment' and give the approximate last treatment date. The WCB will pay for the initial evaluation, but treatment cannot start until the case manager has confirmed that ongoing treatment is supported. The user manual asks you to call the WCB if you have had no decision about care within 5 business days.
The assessment of recovery is a 0 to 10 estimate, where 0 is no recovery and 10 is recovery to pre-injury status, drawing on clinical findings, outcome measures, functional change and psychosocial issues. The intensity score is 0 when average primary resources should bring success, and 1 when complex medical and psychosocial factors need more than average resources.
The user manual expects a return-to-work date unless the worker has been deemed unemployable by the WCB, is awaiting surgery or an MDA with no accommodated duties available, has a non-WCB permanent disability, never missed time from work, is retired, or has no job to return to. A worker who returned gets their actual return date.
