WCB Saskatchewan PPP: Physician's Progress Report

Saskatchewan physicians send this one-page report when an injured worker's condition, diagnosis or treatment changes, about every three weeks while the worker is off work, and within three days of discharge. It updates findings and diagnostic results, rates recovery from 0 to 10 with any reason for delay, and records off-work advice, restrictions and transitional return-to-work plans.

Who completes it

The primary physician completes and signs the whole form and can note who receives a copy. The worker and employer complete no part of it, though items 14 and 15 record whether transitional return to work has been discussed with each of them and arranged.

Submitting and payment

How to submit it
Submit it through a WCB online account or fax the signed form to the numbers printed on it, about every three weeks while the worker is off work and within three days of discharge.
Who pays
The WCB pays fee code 660 for the progress report, plus code 661 when it is submitted through the WCB's online services.

Good to know

  • Complete items 8 to 18 only when you have advised the worker to be off work or to work with restrictions, as item 7 directs.
  • You may attach clinic notes if they include everything the form asks for and cover only the work injury; the WCB says typewritten notes are especially helpful.
  • Item 10 lets you ask the WCB to arrange or expedite diagnostics or a specialist, and the WCB doesn't require a specialist referral to arrange MRI, CT or other tests.
  • If you also counsel the worker on return to work and complete an employer-provided restrictions form at the visit, fee code 640 is billable per 10 minutes.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

How often should I send a PPP to WCB Saskatchewan?

Send it when the worker's condition, diagnosis or treatment changes, about every three weeks while the worker remains off work, and within three days of discharge. Item 13 estimates how long the injury may affect activity, from under 8 days to more than 21 days, and item 18 records the next appointment date.

What fee codes apply to the PPP?

WCB Saskatchewan pays fee code 660 for the Physician's Progress Report and adds code 661 when it is submitted through the WCB's online services. The initial report (PPI) uses codes 650 and 651 instead. Counselling on return to work together with an employer-provided form is billed separately under code 640.

Should the PPP give restrictions or just say off work?

WCB Saskatchewan's support package for physicians says restrictions should be provided in almost all circumstances unless the worker is hospitalized or housebound, and the onus is on the employer to match work to them. Item 12 lists lifting, pushing or pulling, reaching, overhead reaching, turning, walking, stairs, ladders, standing and sitting hours, and environment.
