WCB Saskatchewan CHP: Chiropractor Progress Report

Saskatchewan chiropractors use this two-page report to update the WCB after each block of six treatments and to report discharge. It compares initial and current scores on the functional outcome measure, gives a 0 to 10 recovery rating and records whether the worker returned to regular duties. It also lets you ask the WCB to arrange or expedite diagnostics, a specialist or an assessment team review.

Who completes it

The treating chiropractor completes and signs the whole form. Item 15 asks whether you have contacted the employer about current restrictions, and item 20 lets you start the electronic Practitioner's Return to Work (PRTW) form.

Submitting and payment

How to submit it
Use the WCB's online submission form or the downloadable PDF, signed before mailing or faxing; send it after each block of six treatments, and as a discharge report within seven days of discharge.

Good to know

  • Item 6 records the outcome measure as initial and current raw scores; if none was done at the first exam, the user manual says to enter 0 and explain why in item 21.
  • For a discharge, tick yes in item 8 and give the date; if the worker hasn't returned to regular duties, complete the rest of the form, since care may continue elsewhere.
  • Item 13 asks about other health or non-health factors affecting recovery, and item 9 asks for diagnostic results received since the previous report.
  • If the worker is back at work with restrictions, item 18 asks how long they will last and the anticipated date of full hours and duties.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

How often do chiropractors send the CHP to WCB Saskatchewan?

After each block of six treatments, and as a discharge report within seven days of discharge. The same two-page form serves both purposes: item 8 asks whether the worker is being discharged from treatment and, if so, the discharge date and whether they returned to their regular duties.

Can I ask the WCB to speed up tests or referrals on the CHP?

Yes. Item 14 asks whether you would like the WCB to arrange or expedite a diagnostic test, a specialist appointment, an assessment team review or another service, with space for details. Item 10's management plan also lets you indicate secondary or tertiary treatment alongside medication, physical therapy, massage, a specialist or surgery.

Does the CHP ask about return to work?

Yes. Page 2 asks whether the worker is off work and who advised it, whether a return to work is arranged and by whom, the return date, and current restrictions such as lifting, reaching, stairs, ladders and standing or sitting hours. Item 20 notes the electronic PRTW form needs to be completed one week before the return.
