WCB Saskatchewan CR: International Provider's Report

Care providers outside Canada who treat a worker injured on the job in Saskatchewan use this three-page WCB report to document the worker's progress. The WCB pays the provider's fees, and those of any care providers they refer the patient to, for care related to the workplace injury, as long as it receives regular reports. The report covers diagnosis, findings, treatment, restrictions and return to work.

Who completes it

The treating health care provider completes the whole report, giving their name, clinic details and clinic website along with the patient's WCB claim number, Saskatchewan Health number, date of injury and job type.

Submitting and payment

How to submit it
Return each completed report to the Saskatchewan WCB by email or by mail to the addresses printed on the form, with copies of all test results and specialist reports attached.
Who pays
The WCB pays the provider by cheque or electronic funds transfer at the rates in its Saskatchewan fee schedules, provided it receives regular progress reports.
