WCB Saskatchewan CHI: Chiropractor's Initial Report

Saskatchewan chiropractors send this two-page report to the WCB within three days of starting treatment for a work injury. It records the diagnosis, mechanism of injury and objective findings, a functional outcome measure such as the Roland Morris, QuickDASH, NDI or LEFS, and a 0 to 10 recovery score. The second page covers time off work, current restrictions and return-to-work arrangements.

Who completes it

The treating chiropractor completes and signs the whole form, adding the clinic number and provider number. No other party signs it, but item 16 asks whether you have contacted the employer about current restrictions, when and with whom.

Submitting and payment

How to submit it
Send it within three days of starting treatment, through the online submission form in a WCB online account or by signing the PDF and mailing or faxing it to the WCB.
Who pays
Fees follow WCB Saskatchewan's service fees and fee codes for primary chiropractic service providers (2024 to 2027), and services are billed on the Chiropractor's Billing Form (CHRO).

Good to know

  • For recurrent treatment, the WCB pays for the initial evaluation, but treatment can't start until the case manager confirms ongoing care is supported; call if there's no decision within 5 business days.
  • Objective findings should include quantifiable measures such as range of motion in degrees or percentage, manual muscle testing graded out of 5, SLR, DTR, sensation and limb girth.
  • Item 21 offers the electronic Practitioner's Return to Work (PRTW) form, which can be built and submitted with the CHI and needs completing one week before the return.
  • Later updates go on the Chiropractor's Progress/Discharge Report (CHP), which repeats the outcome measure so initial and current scores can be compared.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

When is the WCB Saskatchewan CHI due?

Within three days of starting treatment for a work injury. Submit it through the online submission form in your WCB online account, or sign the PDF and mail or fax it. After that, progress goes on the Chiropractor's Progress/Discharge Report (CHP), sent after each block of six treatments and within seven days of discharge.

Which outcome measure goes on the CHI?

Item 8 offers the Roland Morris, QuickDASH, QD work module, NDI and LEFS. The CHI/PTI user manual matches them to the region treated: NDI for the cervical spine, Roland Morris for the thoracic and lumbar spine, DASH for upper quadrant injuries and LEFS for the lower quadrant. Enter the raw score, or 0 with an explanation if none was done.

What return-to-work details does the CHI ask for?

Page 2 asks whether the worker is off work and who advised it, how long they may be off, whether a return to work is arranged and by whom, and the return date. It then lists estimated restrictions, marked subjective or objective, such as lifting, reaching, stairs, ladders and standing or sitting hours, and whether you and the worker agree.
