WCB Saskatchewan PTP: Physical Therapy Progress Report

Saskatchewan physical therapists use this two-page report, also available online, to update the WCB on a worker's progress or to report discharge. It compares initial and current outcome measure scores, rates recovery from 0 to 10, and asks whether the worker has been discharged and returned to regular duties. Management, employer contact, work status and current restrictions follow, with an option to start the electronic PRTW.

Who completes it

The treating physical therapist completes and signs the report with the clinic and provider numbers. If you have not contacted the employer about restrictions, item 15 asks why and when you will, since the WCB's standards of care expect every care provider to start return-to-work communication.

Submitting and payment

How to submit it
Submit it through the WCB's online form in a WCB online account, or sign the downloadable PDF and mail or fax it to the WCB at the numbers printed on it.

Good to know

  • When discharging a worker who has returned to regular duties, the online form skips straight to the signature; otherwise complete the rest, because care may continue elsewhere.
  • Present subjective complaints and objective findings in the same quantified format as earlier reports, such as a numerical pain rating, range of motion in degrees and graded muscle testing.
  • Use the general comments to flag ongoing treatment elsewhere, such as a move to tertiary care, when the worker leaves your care.
  • Item 18 lets you state how long restrictions should last, in days or weeks, or give the anticipated date of full hours and duties.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

How does the PTP show a worker's progress?

Item 6 records the self-report outcome measure, such as the NDI, Roland Morris, DASH or LEFS, as both the initial and the current raw score so the change is visible. Item 7 gives your 0 to 10 assessment of recovery, drawing on clinical findings, self-reported change, functional change and possible psychosocial issues.

Can I ask WCB Saskatchewan to expedite a test or specialist on the PTP?

Yes. Item 14 offers a diagnostic test, a specialist or an assessment team review, with an open field to name the type of test or specialist. Item 10's management plan records the services you understand to be part of the plan, including a pending specialist appointment or secondary or tertiary treatment.

What if the worker is back at work with no restrictions?

Answer 'no' to working with restrictions in item 18 and tick 'No restrictions' in item 19 to show the worker is on full hours and duties. If restrictions remain, give their expected duration or the anticipated date of full hours and duties, and mark whether they are subjective or based on functional testing.
