Massage therapists ask WCB Saskatchewan for authorization to treat an injured worker at the primary level with this one-page MCARE form. It identifies the clinic, therapist, worker, claim and area of injury, names the licensed practitioner who referred the worker, and requests up to 5 treatments. The WCB case manager records the funding decision and which report fees it will fund.
The massage therapist completes the request and attaches the referral document from the licensed practitioner. WCB personnel complete the decision sections, marking the request approved, denied or provisionally authorized and adding the case manager's name and date.

Primary Level Authorization to Treat – Massage Therapy (MCARE)
If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Up to 5. The form states the request is not to exceed 5 treatments. If the worker needs more, the WCB funds a progress report where an extension of the originally approved treatment is requested, and the decision box on this form shows whether that report fee is covered.
Yes. The MCARE confirms that a licensed practitioner referred the patient for massage therapy and asks you to name them and attach the referral document. If the referral hasn't reached the WCB, it can provisionally fund treatment pending receipt of the referral document.
The WCB funds treatment before everything is settled: either while adjudication of the claim occurs, or pending receipt of the referral document. The case manager ticks which applies and dates the decision, alongside the choice of report fees the WCB will fund for this worker.
