WCB Saskatchewan TXP: Practitioner's Progress Report

Other accredited treating practitioners use this one-page WCB Saskatchewan form to update the WCB after the initial TXI. It records current complaints, clinical findings, unrelated conditions that may affect recovery, progress toward the functional goals needed to return to work and any impediments to recovery. It also logs treatment dates since the last report, absences, work status and the expected discharge date.

Who completes it

The treating practitioner completes and signs the whole form with the clinic number and care provider number, and can note who receives a copy.

Submitting and payment

How to submit it
Submit it through the WCB's online submission form for the TXP, or download the PDF, sign it, and mail or fax it to the WCB at the numbers printed on it.
Who pays
Treatment for the worker is billed to the WCB on the Therapist's Billing (THER) form.

Good to know

  • Items 8 and 9 record the dates the worker was absent and the dates of treatment since the last report.
  • If the worker is back at work, item 7 asks when they returned; if not, it asks for the expected return-to-work date.
  • Item 5 asks you to identify any impediments to recovery, alongside the conditions not related to the work injury described in item 3.
  • The Practitioner's Discharge Summary (TXD) closes the file with one of six outcome codes once treatment ends.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What is the difference between the TXI, TXP and TXD?

They are WCB Saskatchewan's three generic reports for other accredited treating practitioners. The TXI covers the initial assessment, the TXP reports progress toward treatment goals during care, and the TXD summarizes the worker's functional status and outcome at discharge, including the total number of appointments attended.

What progress does the TXP measure?

Item 4 asks for progress towards treatment goals, defined as the functional abilities required to return to work, which the initial TXI sets in its item 8. The report also asks for current complaints, clinical findings, any impediments to recovery and an updated treatment plan with the frequency of appointments.

Can the TXP be filed online?

Yes. WCB Saskatchewan offers online submission for the TXI and TXP, and the PDF version can be downloaded instead. If you use the PDF, the form asks you to sign it before mailing or faxing, and to put the worker's name and claim or firm number on any correspondence.
