WCB Saskatchewan TXI: Practitioner's Initial Assessment

Other accredited treating practitioners use this generic one-page WCB Saskatchewan form to report a worker's initial assessment. It records the diagnosis, the worker's injury history and symptoms, clinical findings and any unrelated conditions that may affect recovery, then the work-related functional problems, treatment goals and plan. It closes with the worker's work status, next appointment, expected discharge date and appointment frequency.

Who completes it

The treating practitioner completes and signs the whole form with the clinic number and care provider number, and can list who receives a copy. The worker and employer complete no part of it.

Submitting and payment

How to submit it
Use the WCB's online submission form for the TXI, or download the fillable PDF, sign it, and mail or fax it to the WCB at the numbers printed on it.
Who pays
Treatment for the worker is billed to the WCB on the Therapist's Billing (THER) form.

Good to know

  • Item 6 asks you to describe conditions not related to the work injury that may affect recovery, separately from the work-related diagnosis and clinical findings.
  • Treatment goals are framed as the functional abilities the worker needs to return to work, tied to the work-related functional problems identified in item 7.
  • If the worker is not currently working, item 10 asks for the expected return-to-work date; items 11 to 13 cover the next appointment, discharge date and frequency.
  • Progress and discharge go on the companion Practitioner's Progress Report (TXP) and Practitioner's Discharge Summary (TXD).

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who uses the WCB Saskatchewan TXI form?

WCB Saskatchewan lists the TXI among its care provider forms as a generic initial assessment for other accredited treating practitioners. Physicians, chiropractors, physical therapists, occupational therapists and psychologists have their own initial reports (PPI, CHI, PTI, OTI and PSYI), so the TXI serves accredited practitioners outside those groups.

Can the TXI be submitted online?

Yes. The WCB offers an online submission form for the TXI and the TXP, and the fillable PDF can also be downloaded, signed and mailed or faxed. Correspondence with the WCB should carry the worker's name and claim or firm number, and dates on the form are entered month first (MM/DD/YYYY).

What return-to-work details does the TXI ask for?

Item 10 asks whether the worker is currently working and, if not, the expected return-to-work date. Items 11 to 13 add the date of the next appointment, the expected discharge date and the frequency of appointments, while item 8 sets treatment goals as the functional abilities required to return to work.
