Occupational therapists use this one-page WCB Saskatchewan form to report a worker's progress or discharge. It updates the diagnosis, body areas treated, complaints and findings, and compares initial and current recovery scores with an explanation of any delay. If the worker is not discharged, the form becomes a request for extension of treatment covering restrictions, outcome scores, the treatment plan and transitional return to work.
The treating occupational therapist completes and signs it with the clinic and OT numbers and can list who receives a copy. A box at the top records a denied request for extension and its date.
Download the current version from the source each time; forms change without notice.
Occupational Therapist's Progress/Discharge Report (OTP)
If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Use this report. When the worker is not discharged, item 6 directs you to complete items 7 to 18 as a Request for Extension of Treatment: restrictions, work status, outcome scores, treatment plan, frequency, expected weeks to discharge and return-to-work details. The box at the top records a denied extension request and its date.
Tick 'Discharged' and give the date in item 6. The report still records the current diagnosis, body areas treated, subjective complaints, objective findings and the initial and current recovery scores, so the change since the Occupational Therapist's Initial Report (OTI) is visible.
Yes. Items 15 to 17 ask whether transitional return to work has been discussed with the worker and the employer, whether it has been arranged and its start date, and whether there are specific safety or medication concerns. Item 14 estimates how long the injury may affect activity and gives a return-to-work date.
