WCB Sask OTI: Occupational Therapist Initial Report

Occupational therapists report their first assessment of a worker with a WCB Saskatchewan claim on this one-page form, online or as a PDF. It records the injury date, diagnosis, mechanism, subjective complaints, objective findings and treatment goals, a 0 to 10 recovery score and an intensity score. The treatment plan, an outcome measure score, restrictions and transitional return-to-work arrangements complete it.

Who completes it

The treating occupational therapist completes and signs the whole form with the clinic and OT numbers, and can list who receives a copy. The worker and employer complete no part of it, though item 19 records whether transitional return to work has been discussed with each.

Submitting and payment

How to submit it
Submit it through WCB Saskatchewan's online submission form where offered, or download the PDF, sign it, and fax or mail it to the WCB.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What outcome measures does the OTI list?

Item 16 records a self-report score on the Roland Morris, QuickDASH, QuickDASH work module, NDI or LEFS. Item 9 adds your assessment of recovery status from 0, meaning none, to 10, meaning pre-injury, and item 10 an intensity score of 0 or 1.

What return-to-work questions are on the OTI?

Item 14 asks whether you advised the patient to be off work and, if not, whether they should work with restrictions. Item 18 estimates how long the injury may affect activity, from under 8 days to more than 21 days, with a return-to-work date, and items 19 to 21 cover transitional return to work with the worker and employer.

What report follows the OTI?

The Occupational Therapist's Progress/Discharge Report (OTP). It compares initial and current recovery scores and outcome measures, records the discharge date, and becomes a request for extension of treatment when the worker is not ready for discharge.
