Mental health providers and occupational therapists use this one-page MCARE form to ask WCB Saskatchewan for authorization to give primary-level treatment to an injured worker. It identifies the clinic, provider and professional designation, the worker and area of injury, who referred them, the proposed treatment and the expected number of treatments. The WCB faxes back its decision: approved, denied or provisional authorization.
The mental health provider or occupational therapist completes the request and attaches the referral document, unless they are a direct access care provider. WCB personnel record the decision, the date and the customer care facilitator's name.

Primary Level Authorization to Treat – Mental Health and Occupational Therapy (MCARE)
If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Care providers offering primary-level mental health treatment or occupational therapy to a worker with a WCB Saskatchewan claim. The form records the provider's professional designation, the referral source and the proposed treatment, and has an 'Other' option for services outside those two categories.
The form asks you to attach the referral document except where you are a direct access care provider. Either way, you name who referred the worker for the primary-level services you have checked, then describe the proposed treatment and give the expected number of treatments.
It means the WCB will fund treatment until it makes its adjudication decision on the claim. The other outcomes are approved or denied. WCB staff record the decision with the date and the customer care facilitator's name, then fax the form back to the care provider who sent it.
