WCB Saskatchewan PSYP: Psychology Progress Report

Psychologists treating a worker with a WCB Saskatchewan claim use this six-page form for both progress and discharge reports. It records the DSM diagnoses treated, the current GAF, updated clinical information and whether treatment follows the mental health assessment (MHA) recommendations. Most of the form covers function at home, missed appointments, work status, psychological restrictions and return-to-work planning, which it calls the primary goal of treatment.

Who completes it

The treating psychologist completes and signs it. In a secondary or tertiary treatment centre, the work and functional section is completed jointly with the clinic therapist, who also signs, and the report is integrated and sent from within the centre.

Submitting and payment

How to submit it
Submit it through WCB Saskatchewan's online form where offered, or complete the PDF and send it to the WCB's Health Care Services, marked as a progress or discharge report.
Who pays
Fees follow WCB Saskatchewan's fee schedule for psychological health service providers.

Good to know

  • If treatment departs from the MHA recommendations, WCB Health Care Services must be alerted and the change explained; in a treatment centre, the team contacts the Health Care Services manager.
  • Report missed psychology appointments with the number to date and, for tertiary programs, missed exercise appointments with the number and reason.
  • Describe psychological restrictions using the WCB List of Restrictions information sheet, saying what the worker needs to avoid rather than naming a job position or location.
  • For each reporting period, state the goals set for function at home and daily tasks, and whether each goal was met.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Is the PSYP used for discharge as well as progress?

Yes. Check either 'Progress report' or 'Discharge report' at the top, or delete the one that does not apply, and give the date of discharge on a discharge report. Both versions record the DSM diagnoses treated, the current GAF from DSM-IV-TR and any clinical information that has changed since the last progress report.

What does WCB Saskatchewan expect about return to work in psychology reports?

The form states that supporting the worker to stay at work or return is the primary goal of mental health treatment. It asks for return-to-work factors and restrictions, whether the MHA's return-to-work recommendations are being followed, whether the employer has been contacted, whether workplace visits for live exposure are needed and whether the worker and employer accept the plan.

What if the worker is missing appointments?

The PSYP asks whether the worker has missed any scheduled psychology appointments and how many to date, and, for workers in tertiary programs, whether they have missed exercise appointments, with the number and the reason. External non-claim issues and additional risk factors that may delay recovery are reported separately at the end.
