WCB Saskatchewan MCARE: Hearing Loss Funding Request

Hearing service providers use this four-page WCB Saskatchewan form to request funding for a worker with a work-related hearing loss claim. Part 1 covers new or replacement hearing aids, the reasons for replacement and the invoicing codes. Part 2 covers repairs after the warranty, supplies such as receivers and ear molds, and service visits beyond the four per year that are pre-approved.

Who completes it

The hearing service provider, an audiologist or hearing instrument practitioner, completes both parts with the clinic and provider numbers and signs where the form asks for the care provider's signature. The WCB completes the 'WCB response' boxes, marking each request approved or denied.

Submitting and payment

How to submit it
Send it to the WCB using the fax or email contacts printed in the form's header; the WCB returns its decision in the response box with the customer care facilitator's name and the date.
Who pays
Approved items are paid under WCB Saskatchewan's hearing service fees and fee codes, such as 202 for the hearing aid, 213 for fitting and first-year visits, 205 for repairs and 214 for servicing, billed on the Audiology's Billing (AUD) form.

Good to know

  • A change in hearing justifies replacement only at 20 dB at three or more frequencies between 500 and 4,000 Hz, and the change must be related to the work injury.
  • The first 36 months from purchase are covered by warranty; repair code 205 is billable only after it expires, and older aids need authorization under age and repair-history rules.
  • Attach the manufacturer's document stamped 'not for payment' with the invoice, and describe the new aid's manufacturer, model, style and warranty period, which must exceed three years.
  • If the worker upgrades to a mid-range or premium model, the form asks you to confirm they know the WCB pays only up to its fee schedule amount.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

When will WCB Saskatchewan replace a worker's hearing aids?

The MCARE lists the accepted reasons, ear by ear: improper amplification for the hearing loss, improper fit causing feedback, a work-related change in hearing of 20 dB at three or more frequencies, a style that no longer suits the worker (for example because of dexterity), repairs that are no longer cost-effective, or loss or damage. Each request needs WCB approval.

How many hearing aid service visits are pre-approved each year?

Four. Part 2, section D asks for authorization when a service visit would exceed the pre-approved four visits per year, with the visit number and the reason for the extra visit. Servicing under fee code 214 is billable only after the first year, which the WCB prepays through the fitting code 213.

Do ear molds and receivers need WCB approval?

Only beyond set limits. Part 2, section C asks for authorization for ear molds under fee code 215 when they exceed one mold per ear every two years, and for a receiver needed after the warranty has expired. Give the expected cost, and the WCB marks the request approved or denied in its response box.
