Saskatchewan medical forms for physicians and therapists

All 36 verified Saskatchewan forms: WCB reports, SGI driver and injury forms, SAIL health programs, Saskatchewan Blue Cross, 3sHealth and accessible parking.

Saskatchewan has 36 verified forms in this library, all in English, and 21 come from WCB Saskatchewan. Physicians start with the Physician's Initial Report (PPI) for every work-related injury, including no-time-loss injuries, then send the Progress/Discharge Report (PPP) when things change. SGI handles auto injury claims but publishes no standard practitioner report form. Commercial drivers need SGI's Commercial Driver's Licence Medical Report (3167), and SAIL runs the insulin pump and nutritional products programs.

Submit the PPI within three days of the assessment; WCB Saskatchewan adds a fee when it arrives through WCB online services. SGI expects a written report within six days of a written request, and its permanent impairment forms come attached to its request letters. For drug coverage, the library has Saskatchewan Blue Cross's special authorization form (CLMS-1006). It lists no Saskatchewan income assistance or student aid form yet. Federal forms (T2201, CPP disability, EI) apply here as everywhere; see the federal hub.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Does SGI have a standard injury report form?

No published one. SGI has no public forms page for health care providers and no standard injury report form. Practitioners must give SGI a written report within six days of a written request, under section 168 of the Act. SGI's permanent impairment forms, such as the Permanent Impairment Description form and Range of Motion Charts, arrive attached to its request letters.

When is the WCB Saskatchewan PPI due?

Within three days of the assessment, for every work-related injury, including injuries with no time loss. The PPI confirms disability for wage-loss benefits. Submit it through a WCB online account, which adds a fee on top of the report fee, or by fax. Follow up with the PPP about every three weeks while the worker is off work, and within three days of discharge.

Who pays for an SGI commercial driver medical?

The driver. The Commercial Driver's Licence Medical Report (3167) states that physicians cannot bill SGI. A physician, NP or occupational health nurse completes it before testing for Class 1 to 4 licences, then every 5 years from age 18 to 45, every 3 years from 46 to 65 and yearly from 66. The PDF has no fillable fields.

Who completes a Saskatchewan accessible parking application?

A physician, NP, occupational therapist, physical therapist or chiropractor completes Section 2 for people who can't walk more than 50 metres unassisted without great difficulty or danger. The applicant mails it or brings it to SaskAbilities within 3 months; email and fax aren't accepted because original signatures are required. Renewing a permanent permit needs only the applicant's self-declaration.
