Saskatchewan PTAP: Pediatric Out-of-Province Travel

A Saskatchewan-licensed specialist completes this two-page application to establish that a child 16 or younger needs medical treatment outside the province that is not provided in Saskatchewan. The Pediatric Out-of-Province Travel Assistance Program then considers eligible travel, meal and accommodation expenses. Applications go forward only when the Provincial Department Head of Pediatrics recommends them, and approval is not guaranteed.

Who completes it

The referring specialist, who must be licensed in Saskatchewan, completes Sections A to F (patient, specialist, out-of-province hospital and physician, treatment, availability and declaration) and signs. The Provincial Head of Pediatrics, a ministry medical consultant and the Director of Insured Services complete the internal-use page.

Submitting and payment

How to submit it
Email the completed form to the Saskatchewan Health Authority pediatric travel assistance address printed on it; once the Provincial Head of Pediatrics signs, the form goes to the Ministry of Health's travel assistance program.

Good to know

  • If the required service is available in Saskatchewan, providers may still refer out of province, but the patient will not be eligible for PTAP.
  • The ministry is not obligated to reimburse travel for out-of-province care that was not approved through this application process.
  • Section E asks whether the treatment is an accepted standard of care, whether it is available in Saskatchewan, and how many nights of accommodation are needed.
  • Section D records the diagnosis, the recommended treatment or procedure, and the admission, estimated discharge and out-of-province treatment dates; the current version is dated July 2025.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.
