Saskatchewan Blue Cross: Initial Disability Statement

Saskatchewan Blue Cross requires this medical statement as one of the forms for a disability benefits claim, alongside the employee's statement, education and work history, and the employer's statement and job description. A Disability Case Manager reviews the medical information, functional limitations and job demands within 5 to 10 business days of receiving all required information.

Who completes it

The employee prints, completes and signs Section 1, then asks their doctor to complete the rest of the form. The doctor, or applicable medical provider, completes Section 2 and includes copies of test results and specialist reports.

Submitting and payment

How to submit it
Saskatchewan Blue Cross accepts disability claim documents by mail, fax or the secure Member Portal and asks that completed forms not be emailed. The employee is responsible for following up to make sure the doctor's medical information is submitted.
Who pays
The patient is responsible for any fees for completing the form, as stated on the form and in Saskatchewan Blue Cross's employee guide.

Good to know

  • If the patient has returned to work or the disability will last less than 4 weeks, stop after the surgery questions and sign; otherwise complete all pages.
  • Unattached test results are interpreted as tests not performed, and requests for medical and health information exclude genetic test results.
  • After approval, Saskatchewan Blue Cross may request periodic medical updates, including by forwarding another Initial Disability Insurance Medical Statement for the physician to complete.
  • If a claim is declined, new medical information is required for an appeal and must arrive within 90 days of the decline letter; the patient pays for it.

Fill it with Vero

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
