WCB Manitoba forms for doctors, chiropractors and physiotherapists

First and progress reports for doctors, chiropractors and physiotherapists, plus opioid management, dental and athletic therapy reports for WCB Manitoba claims.

WCB Manitoba is the province's workers' compensation board. Physicians complete the Doctor First Report after a workplace injury, covering fitness for modified work and restrictions, then the Doctor Progress Report as the worker recovers. Chiropractors use the Chiropractor First Report (4C) and Progress Report (7C). Physiotherapists use the Physiotherapy Initial Report (PRI) and the Progress/Discharge Assessment (PPDA). Dentists, athletic therapists and opioid prescribers have their own reports.

WCB Manitoba pays a fee for each of these reports under its service codes. Physician, chiropractor and opioid management reports are faxed, using the number printed on the form. Physiotherapists can use the eHealth Portal, which bills automatically for clinics below the 53rd parallel, and athletic therapists report online only. The French resources page gives French titles for most forms but links the same English PDFs. Vero Documents fills these fillable PDFs from the visit note and the chart.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Which forms does a doctor complete for WCB Manitoba?

Physicians complete the Doctor First Report after the first visit for a workplace injury. It covers fitness for modified work and restrictions. The Doctor Progress Report then covers recovery and return-to-work capacity, including at discharge. Both are fillable PDFs faxed to WCB Manitoba, and each has its own report fee. Optometrists bill these same reports, and narrative reports have a separate fee.

When does WCB Manitoba need a progress report?

For chiropractors and athletic therapists, WCB Manitoba's fee manual says progress reports are needed only when WCB asks or after a notable change in recovery. Chiropractors use the Chiropractor Progress Report (7C) for progress and discharge, and athletic therapists report through the eHealth Portal. Physicians use the Doctor Progress Report for progress and discharge.

How do physiotherapists submit WCB Manitoba reports?

Through the eHealth Portal or with the PDF. Clinics below the 53rd parallel that submit through the portal are billed automatically. Physiotherapists file the Physiotherapy Initial Report (PRI) and the Progress/Discharge Assessment (PPDA). When care needs to continue beyond the authorized period, they send an application for additional treatment, which can also be made in the portal.

What opioid forms does WCB Manitoba use?

Physicians and nurse practitioners prescribing opioids on a claim complete the Initial Opioid Management Report (OMIR) and follow up with the Opioid Management Progress Report (OMPR). The prescriber and the worker also sign an opioid treatment agreement. WCB Manitoba pays physicians and NPs a separate fee for each report.
