WCB Manitoba Doctor Progress Report

After the Doctor First Report, Manitoba physicians use this one-page form to update the WCB on a worker's recovery or discharge. It asks for an ICD code and any change in diagnosis, subjective complaints and objective findings, whether recovery is satisfactory and what is impeding it, tests performed, consultant referrals, the treatment plan and next visit, and the worker's capacity for regular or modified duties.

Who completes it

The treating physician completes and signs the whole form; nurse practitioners bill progress/discharge reports under code 0210, and optometrists bill the doctor's progress report under 3025. The worker and employer do not complete any part.

Submitting and payment

How to submit it
Fax the completed form to the WCB at the number printed on it, attaching test results where relevant. Send a progress report when the WCB requests one or when the worker's recovery changes notably.
Who pays
WCB Manitoba pays physicians the progress/discharge report fee under service code 3025, with narrative reports billed separately under 3026.

Good to know

  • The fee is billable for a completed form, an incomplete form with attached chart or clinic notes containing similar information, or a facility outpatient summary sheet.
  • If no progress report is sent for a second or subsequent visit, physicians can bill the follow-up visit administrative fee (5562) instead.
  • Photocopy charges 0015 and 0016 can be added when the report is accompanied by test results or diagnostic imaging; the chart note preparation fee (0017) cannot.
  • WCB Manitoba expects certificates it requires to be provided at no cost to the worker, and invoices must reach the WCB within 12 months.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Do I need to send a WCB progress report after every visit?

No. WCB Manitoba's service code manual says progress or subsequent reports are not required for every follow-up visit. Submit one when the WCB requests it or when there is a notable change in the worker's recovery. For visits without a report, physicians can bill the follow-up visit administrative fee instead.

Can I attach chart notes instead of completing the whole form?

For billing purposes, yes. The progress report fee (3025) is payable for a completed form, or for an incomplete form with attached chart or clinic notes from the visit containing information similar to what the report asks for. An outpatient summary sheet generated by a healthcare facility also qualifies.

Is the Doctor Progress Report available in French?

WCB Manitoba's French resources page lists it as Rapport du médecin sur l'évolution du patient, but it links to the same English PDF. The fillable English form is the one to complete and fax, with the worker's claim number at the top.
