WCB Manitoba Opioid Management Progress Report (OMPR)

After the initial opioid management report, the prescribing physician or nurse practitioner uses this two-page form to update the WCB on a worker's opioid treatment. It repeats the pain and function ratings so change can be tracked, and adds side-effect severity ratings and a checklist of adverse opioid-related factors such as unsanctioned use, opioid seeking or withdrawal symptoms.

Who completes it

The prescribing physician or nurse practitioner completes and signs the report, based on examination and the worker's reported pain and function. The worker does not sign this form.

Submitting and payment

How to submit it
Fax the completed report to the WCB at the number printed on the form, with the clinic name, prescriber's name and the date of the next visit filled in.
Who pays
WCB Manitoba pays physicians service code 8115 and nurse practitioners code 0021 when a completed opioid management progress report is submitted.

Good to know

  • Rate nausea, constipation, sweating, dry mouth, depressed mood, cognitive deficits, fatigue or drowsiness and overall side effects as none, mild, moderate or severe.
  • Flag any adverse factors: altering the route of delivery, unsanctioned use, withdrawal symptoms, accessing opioids from other sources, opioid seeking and social deterioration.
  • Pain over the past week and function at home, at work, walking, sleeping and overall are scored 0 to 10, matching the initial report for comparison.
  • If a follow-up visit has no opioid or doctor's progress report, physicians may bill the WCB's follow-up visit administrative fee instead.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What changes between the OMIR and the OMPR?

Both ask for symptoms, the diagnosis accounting for the pain, pain and function scores, current medications, work capabilities and the treatment plan. The progress report drops the Opioid Risk Tool and treatment agreement questions and adds side-effect ratings and a checklist of adverse opioid-related factors.

Is the opioid progress report fee guaranteed if the claim is denied?

WCB Manitoba's fee notes mark the initial opioid report fee as a guaranteed expense, but not the progress report fee. Physicians bill 8115 and nurse practitioners 0021, each payable when a completed opioid management progress report is submitted.

Is the OMPR available in French?

WCB Manitoba's French resources page lists it as Rapport sur l'évolution de la gestion des opioïdes and links to the same English PDF, so the fillable English form, marked OMPR beside the claim number box, is the one to complete.
