WCB Manitoba Initial Opioid Management Report (OMIR)

When a worker with a WCB Manitoba claim is prescribed opioids, the physician or nurse practitioner sends this two-page initial report. It captures the diagnosis that accounts for the pain, average pain over the past week on a 0 to 10 scale, five function ratings, the Opioid Risk Tool score, whether a treatment agreement is in place, current medications and work capabilities.

Who completes it

The prescribing physician or nurse practitioner completes and signs the report. The worker's part is the separate Opioid Treatment Agreement, which the worker and prescriber both sign and which goes to the WCB with this form.

Submitting and payment

How to submit it
Fax the report to the WCB at the number printed on the form, with a copy of the signed Opioid Treatment Agreement. Do not forward the completed Opioid Risk Tool questionnaire; report only its score.
Who pays
WCB Manitoba pays physicians service code 8114 and nurse practitioners code 0020 for a completed initial opioid management report, a guaranteed fee paid even if the claim is disallowed.

Good to know

  • The WCB requires the opioid treatment agreement between worker and prescribing doctor before it will consider paying for opioid medication.
  • Function is scored from poor (0) to normal (10) for home, work, walking, sleeping and overall function, based on the patient's own report.
  • List every current medication with strength in milligrams, frequency and duration, including new prescriptions, and include further investigations or consultations in the treatment plan.
  • Follow-up reports use the Progress Opioid Management Report (OMPR), billed under 8115 for physicians and 0021 for nurse practitioners.

Frequently asked questions

Do I send the Opioid Risk Tool to WCB Manitoba?

No. The report asks whether the Opioid Risk Tool was completed and for the score, but it says not to forward the completed ORT questionnaire. It does ask you to send a copy of the Opioid Treatment Agreement to the WCB with the report.

Who can complete the WCB Manitoba initial opioid report?

Physicians and nurse practitioners. WCB Manitoba's fee schedules include an opioid management initial report fee for both: service code 8114 for physicians and 0020 for nurse practitioners, each billable when a completed initial report is submitted and paid even if the claim is later disallowed.

Is the OMIR available in French?

WCB Manitoba's French resources page lists it as Formulaire de rapport initial de gestion des opioïdes, but it links to the same English PDF. Complete the English fillable form, which has OMIR printed beside the claim number box.
