WCB Manitoba Athletic Therapy Reports (eHealth Portal)

Athletic therapists treating a worker with a WCB Manitoba claim file their initial and progress/discharge reports online rather than on a paper form. The eHealth Portal, open to clinics below the 53rd parallel, also takes visit invoices, requests for additional treatment or multi-site visits and requests to consult a WCB physiotherapy consultant, and it bills report fees automatically when a report is submitted.

Who completes it

The treating athletic therapist completes and submits their own reports in the portal; submission confirms agreement with the content and replaces a signature, and the submitter's user ID appears on the report. A clinic administrator registers users and manages their access.

Submitting and payment

How to submit it
Submit through the WCB eHealth Portal, where reports and invoices cannot be filed without the worker's claim number. WCB requests for reports arrive only in the portal's Reports in Progress tab, so log in regularly.
Who pays
WCB Manitoba pays the initial report fee (service code 0204) whether the claim is later accepted or disallowed, and the progress/discharge report fee (0205) when a completed report is submitted.

Good to know

  • One initial assessment and 18 follow-up treatments are approved at first; treatment beyond 18 visits needs pre-approval, and a request for additional treatment is billed under 8116.
  • Progress reports are not needed for every visit: send one when the WCB asks or when there is a notable change in the worker's recovery.
  • To create a discharge report, complete a progress report and answer Yes to the question asking whether the worker is discharged; the portal generates it automatically.
  • Unsaved portal work is lost after 30 minutes of inactivity, and the WCB asks clinics to keep their own copies of submitted reports.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Is there a paper WCB Manitoba report form for athletic therapists?

WCB Manitoba's resources and forms page lists paper reports for physiotherapy, chiropractic, dentistry and physicians, but none for athletic therapy. Athletic therapists in clinics below the 53rd parallel submit initial and progress/discharge reports through the eHealth Portal, which bills the report fee automatically when the report is submitted.

How many athletic therapy visits does WCB Manitoba approve?

One initial assessment and 18 follow-up treatments are approved at first, and pre-approval is required beyond 18 treatments. Requests for additional treatment can be sent through the eHealth Portal and are billed under service code 8116, which cannot be combined with the progress/discharge report fee.

How do I find a worker's claim number in the eHealth Portal?

Go to Manage Patients, then the Search WCB Claim tab. Enter the claim number if the worker has it, or search by last name, year of birth and date of incident. If no claim appears, one may not have been started, and the worker should contact the WCB before their scheduled treatments.
