WCB Manitoba Physiotherapy Progress/Discharge (PPDA)

Manitoba physiotherapists use this one-page form for both progress reports and the discharge report on a WCB claim, ticking one at the top. It updates the diagnosis, current complaints, self-assessment scores and impairments, asks whether recovery is satisfactory, and for discharge records the worker's status and the reason for discharge, along with any home program and work capabilities.

Who completes it

The treating physiotherapist completes and signs the form. The worker does not sign, but the form asks whether findings and recommendations were discussed with them and whether a home program was provided.

Submitting and payment

How to submit it
Submit it through the WCB eHealth Portal (clinics below the 53rd parallel) or fax the fillable PDF. In the portal, a discharge report is created by completing a progress report and answering Yes to the question asking whether the worker is discharged.
Who pays
WCB Manitoba pays the progress/discharge report fee under service code 0203 when a completed report on the WCB form is submitted.

Good to know

  • Progress reports aren't needed after every visit: send one when the WCB requests it or when the worker's recovery changes notably.
  • In the eHealth Portal, WCB report requests appear only in the Reports in Progress tab, with no fax, mail or email copy, so check it regularly.
  • Use at least two self-assessment tools and record their scores, such as the NPRS, Roland Morris, Neck Disability Index, LEFS, DASH or health status disability.
  • The progress/discharge report fee cannot be combined with the fee for a request for additional treatment (8116), which has its own form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

When does WCB Manitoba want a physiotherapy progress report?

Not after every visit. The service code manual says progress or subsequent reports should be submitted when the WCB requests them or when there is a notable change in the worker's recovery. The report fee (0203) is billable only when a completed report on the WCB form is submitted.

How do I submit a physiotherapy discharge report in the eHealth Portal?

Complete a progress report and select Yes when asked whether the worker is discharged. The portal then creates the discharge report automatically. On paper, tick Discharge Form at the top of the PPDA and complete the status at discharge and the reason for discharge.

Is the PPDA available in French?

WCB Manitoba's French resources page lists it as Rapport sur l'évolution/la fin de la prise en charge du patient en physiothérapie, linking to the same English PDF. The English form carries PPDA beside the claim number box.
