SR-2100 Rapport d'évaluation médicale (Québec)

Québec's Ministère de l'Emploi et de la Solidarité sociale uses the SR-2100 report to decide whether a person has temporary, prolonged or permanent health constraints, for last-resort assistance under the Social Assistance or Social Solidarity programs. The professional records the diagnosis, severity and prognosis, codes the functional limitations and their expected duration, and says whether they prevent work or employment-preparation activities.

Who completes it

Physicians, specialized nurse practitioners, psychologists, social workers and couple and family therapists may complete Sections 2 to 5 and sign. The person fills in Section 1 and signs the Section 6 consents, which are printed in French and English.

Submitting and payment

How to submit it
With the person's consent, the professional can send the form directly to the ministry; otherwise the person submits it through the Mon dossier online service for social assistance, by mail or in person at a Services Québec office.

Good to know

  • Functional limitations are entered as codes from the list on the back, covering mobility, effort tolerance, daily living, communication, elimination, behaviour, and affective and cognitive mental functions.
  • Duration choices are temporary (number of months), prolonged (12 months or more, with possible improvement) or permanent, and the ministry can reassess the health constraint as needed.
  • Québec.ca says the interruption of activity must last at least one month. If the person also has medical needs, the SR-0040 Certificat médical must be completed too.
  • A second consent lets the professional exchange additional information with the ministry; without it, the ministry must contact the person first, which can slow the file.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can complete the SR-2100 form?

The back of the form lists the authorized professionals: physicians, specialized nurse practitioners, psychologists, social workers, and couple and family therapists. Section 5 records the professional's licence number and profession, whether they provide the person's clinical follow-up and since when, and whether the ministry's evaluator may contact them.

Is the SR-2100 available in English?

The form itself is in French, with the person's consents printed in both French and English. The form notes that a courtesy English translation is available on Québec.ca, which can help when going through the form with an English-speaking patient.

What employment activities does SR-2100 ask about?

Besides asking whether the limitations prevent the person from entering the labour market, Section 3 asks whether they prevent activities that prepare for work, such as literacy training, secondary or technical training, job-readiness projects of at least 20 hours a week, or employment assistance services.
