Handicapped Children Supplement Reassessment (LPF-630)

Retraite Québec uses these condition-specific reassessment reports to check whether a child still qualifies for the Supplement for Handicapped Children. There are four: physical impairments (LPF-630A1), food allergies, hearing, and mental function disabilities including language disorders. Each has a parent's part, a consent form and a professional's part that attests to the severity and extent of impairments, the resulting disabilities and the care required.

Who completes it

The professional's part is completed by the health professional most familiar with the child's current condition, a member of a professional order who has assessed or seen the child in the last 12 months. The parent completes the parent's part, Section 1 of the professional's part and the consent.

Submitting and payment

How to submit it
The professional returns the completed form to the parent as soon as possible, and the parent sends everything to Retraite Québec online, through My Account or by mail, within 120 days of the date the form was sent.
Who pays
Any fee the professional charges is at the parents' expense, according to the reassessment form's instructions.

Good to know

  • If the parent's or professional's part is not received within 120 days, the supplement stops from the following month, though Family Allowance continues.
  • A complete, detailed assessment report from the last 12 months can replace the professional's part, according to the physical impairments report's instructions.
  • The physical impairments report covers 14 condition areas, from failure to thrive and motor impairment to epilepsy and cancer, plus complications, chronic sequelae and a mandatory prognosis.
  • Entitlement to the supplement ends if the reassessment is refused or not followed up, which is why the form asks the professional to return it to the parent quickly.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What happens if the SEH reassessment report is not returned?

The parent has 120 days from the date Retraite Québec sends the form. If the parent's part or the professional's part is still missing after that, the supplement stops as of the month after the deadline. Family Allowance payments continue even if the supplement stops.

What are the four SEH reassessment reports?

Retraite Québec publishes reassessment reports for physical impairments, food allergies, hearing, and mental function disabilities including language disorders. Each has the same structure: a parent's part, a consent to release medical, psychosocial and education-related information, and a professional's part completed by the professional who knows the child's current condition best.

Can an assessment report replace the professional's part?

For the physical impairments report, yes. Its instructions say that if the parent obtained a complete and detailed assessment report from the professionals within the last 12 months, they can enclose it with the application and do not need to provide the professional's part.
