PEI Social Programs Health Report (AccessAbility)

Prince Edward Island's social development department uses this health report to assess program eligibility, employability and support planning, and it is attached to AccessAbility Supports Policy 1.1 Eligibility. The practitioner describes the medical conditions and their expected duration, judges whether the person can work, and rates restrictions in daily living using four classes. The department may combine it with its own assessment to confirm eligibility.

Who completes it

A health care practitioner (MD, NP or specialist) completes Sections 3 to 6, signing with a registration number and stating whether they have followed the patient for 6 months or more. The applicant completes Section 1 and signs the Section 2 authority to release information.

Submitting and payment

How to submit it
Email a scanned, signed copy with the subject line Health Report to the department's social programs inbox, or mail or fax it to the regional office ticked on the last page.

Good to know

  • Section 4 (Employability) is completed only if the practitioner says the person cannot work, and Section 5 (Disability Assessment) only if they consider the person to have a disability.
  • Restrictions in seven ADLs and seven IADLs are rated from Class 1 (0 to 24%) to Class 4 (75% or more), and a lack of financial resources must not be counted.
  • AccessAbility Supports requires a disability diagnosed by a medical practitioner in an applicant under 65, and eligibility is reviewed at least once every 12 months.
  • If the practitioner has followed the patient for less than 6 months, Section 6 asks whether they reviewed previous medical records.
