NWT Form D (NWT9386): Disability Assessment

The NWT Department of Education, Culture and Employment uses Form D to confirm a disability for the Income Assistance for Seniors and Persons with Disabilities program. The practitioner rates the impairment as permanent, long-term, episodic or short-term and describes how it restricts daily living and the ability to meet basic financial needs. The form goes directly from the practitioner to the regional ECE Service Centre.

Who completes it

A medical practitioner or nurse practitioner completes Section 2, initialling each page and certifying with their registration number. A registered nurse licensed in the NWT may complete Section 3 only when there is no access to a medical practitioner or nurse practitioner in the next 3 months, and the applicant completes Section 1.

Submitting and payment

How to submit it
The practitioner sends the completed form through the GNWT Secure File Transfer service to the applicable regional ECE Service Centre: Beaufort-Delta, Dehcho, Sahtú, North Slave or South Slave.

Good to know

  • A person qualifies when an impairment significantly restricts daily living and meeting basic financial needs permanently, or persistently for a continuous period expected to last more than 12 months.
  • The applicant must fill in their last name, first name and date of birth before the health care professional completes the form, and must sign the consent.
  • A diagnosis is not required if unknown; the form focuses on how the condition impairs the person, expected recovery time, available treatment and whether they must live with a parent or caregiver.
  • Daily-living activities in the definition include maintaining employment, managing finances, taking part in community activities and getting around without a caregiver or family member.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Can a registered nurse sign NWT Form D?

Yes, but only in Section 3 and only when the person has no access to a medical practitioner or nurse practitioner in the next 3 months. The nurse must be licensed in the NWT, base the answers on the patient chart, and certify that a physician or nurse practitioner cannot complete the form.

Is Form D available in French?

Yes. ECE publishes a fillable French version, Formulaire D, Évaluation de l'incapacité, alongside the English form NWT9386. Both have the same sections: the applicant's information and consent, the medical or nurse practitioner's assessment, and the registered nurse's alternative assessment.

What does episodic mean on Form D?

The form defines episodic as a prolonged, lifelong condition with unpredictable episodes of illness and disability, often followed by periods of wellness. Episodic or recurrent is a separate choice from long-term, which means the impairment affects the person continuously for the next 12 months.
