ODSP Medical Review Package Part A (OHIP K057)

Ontario's Disability Adjudication Unit uses the Medical Review Package to check whether an ODSP recipient with a medical review date still meets the definition of a person with a disability. Part A is mandatory and collects current information about the conditions identified in the previous disability determination. A medical review is not a new application, and the answers at the end of Part A decide whether Part B is needed.

Who completes it

A physician, nurse practitioner, registered nurse, psychologist, psychological associate or optometrist completes Part A. The recipient may add the optional Self Report form, and the package includes a summary of the original disability decision for reference.

Submitting and payment

How to submit it
The recipient mails the original completed forms to the Disability Adjudication Unit in the envelope provided, within 90 calendar days; extensions can be requested from the unit, which accepts only original forms.
Who pays
Physicians bill OHIP fee code K057 for Part A, while other health care professionals send the ministry an invoice using its posted invoice form.

Good to know

  • Because the ministry does not assess the patient in person, the forms are its main evidence: complete every section, give the duration of each condition and impairment, and sign all pages.
  • Attach supporting information such as clinical notes, hospital reports or consultation reports. The Disability Adjudication Unit may contact the professional for more medical information.
  • The review looks at changes to the original conditions and any new medical issues; conditions not listed in Part A are covered in Part B.
  • Invoices from other professionals must show their name, profession and contact details, the patient's name, date of birth and social assistance member ID, and the section completed.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can complete Part A of the ODSP medical review?

Part A can be completed by a physician, optometrist, psychologist, psychological associate, nurse practitioner or registered nurse. The same professions can complete Part B's Health Status Report, while a wider group, including physiotherapists and occupational therapists, can complete the Activities of Daily Living section.

How long does a patient have to return the ODSP Medical Review Package?

The patient has 90 calendar days to mail back the completed original forms in the envelope provided. If more time is needed, they can contact the Disability Adjudication Unit to ask for an extension. The unit accepts only original forms.

Is the ODSP medical review a new application?

No. The ontario.ca guidance says a medical review is not a new application. It focuses on changes to the conditions identified at the original disability determination and on any new medical issues, and specialized staff at the Disability Adjudication Unit decide whether the person remains medically eligible.
