ODSP Medical Review Part B: Health Status and ADL

When the questions at the end of Part A show that an ODSP recipient has medical conditions not listed there, the Disability Adjudication Unit needs this second part of the Medical Review Package. It has two sections, the Health Status Report (HSR) and Activities of Daily Living (ADL), and it helps decide whether the recipient still meets ODSP's definition of a person with a disability.

Who completes it

The Health Status Report can be completed by a physician, nurse practitioner, registered nurse, psychologist, psychological associate or optometrist. The ADL section can also be completed by a chiropractor, physiotherapist, occupational therapist, social worker, audiologist or speech language pathologist.

Submitting and payment

How to submit it
The recipient mails the original Part B with Part A to the Disability Adjudication Unit in the envelope provided, within 90 calendar days, and can ask the unit for an extension.
Who pays
Physicians bill OHIP K058 for both the HSR and ADL, K059 for the HSR only and K060 for the ADL only, while other professionals send the ministry an invoice naming the section completed.

Good to know

  • Complete Part B only when the answers at the end of Part A say it is needed; separate fee codes exist for completing the HSR alone or the ADL section alone.
  • Give the duration of each condition and impairment, sign every appropriate page, and attach clinical notes or hospital and consultation reports that support the conditions described.
  • Non-physician invoices must include the patient's name, date of birth and social assistance member ID and state whether the HSR, the ADL section or both were completed.
  • The optional Self Report form, updated in August 2021, lets the patient describe in their own words how their disability affects their life.

Frequently asked questions

When is Part B of the ODSP medical review required?

Only when the questions at the end of Part A say so. Part B covers medical conditions that are not listed in Part A, which holds the conditions identified at the original disability determination. Its two sections, the Health Status Report and Activities of Daily Living, can be billed together or separately.

Which OHIP codes apply to ODSP Part B?

Physicians bill K058 when they complete both the Health Status Report and the Activities of Daily Living section, K059 for the Health Status Report only, and K060 for the ADL section only. Part A is billed separately under K057, and other professionals invoice the ministry instead of OHIP.

Can a physiotherapist or occupational therapist complete ODSP Part B?

Yes, for the Activities of Daily Living section only. Chiropractors, physiotherapists, occupational therapists, social workers, audiologists and speech language pathologists can complete the ADL section, while the Health Status Report is limited to physicians, nurse practitioners, registered nurses, psychologists, psychological associates and optometrists.
