ODSP Form 3183E: Vision Care Exceptional Circumstances

Ontario's Ministry of Children, Community and Social Services uses form 3183E when a vision care provider needs pre-authorization for medically necessary items that are not in the vision care fee schedule, such as medically necessary contact lenses. The provider records the prescription, lens type and itemized costs, then explains why basic eyeglasses cannot meet the client's needs. A ministry director records the maximum amount approved.

Who completes it

The vision care service provider, such as an optometrist or optician registered as a supplier, completes Sections C to E with their licence and supplier numbers, the prescription and the justification. The recipient, or someone lawfully authorized, signs the Section B declaration and consent.

Submitting and payment

How to submit it
The provider mails the claim, with a copy of the Vision Care Authorization form and supporting documents, to the ministry's ODSP Vision Care (Exceptional Circumstances) Program at the address on the form, and must get pre-authorization before providing the service.
Who pays
The form sets no completion fee; Section F records the maximum amount a ministry director approves for the requested items.

Good to know

  • Contact lenses are considered when medically necessary for corneal abnormalities, astigmatism not correctable with spectacle lenses, refractive error greater than 8 diopters, anisometropia or another specified reason.
  • Section D records sphere, cylinder, axis, prism, base and add for each eye, the lens type, and itemized costs for frames, lenses, contact lens materials and fitting.
  • Supporting documents must be attached for an exceptions request to be considered, including a copy of the Vision Care Authorization form.
  • The application will not be approved without the signed declaration and consent; for a child under 16, the social assistance applicant or recipient or another lawful custodian signs.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What counts as an exceptional circumstance for ODSP vision care?

Section E asks the provider to outline the client's special needs and explain why they cannot be met with basic eyewear as set out in the ministry's vision care schedule. Medically necessary contact lenses are the main example, with corneal abnormalities, uncorrectable astigmatism, high refractive error and anisometropia listed on the form.

Does the provider need approval before dispensing?

Yes. The ministry's form description says service providers must obtain pre-authorization before providing these services to clients. A ministry director signs Section F and records the maximum amount approved, so the decision comes before the requested items are supplied.

Does form 3183E open in a web browser?

No. It is an XFA-based e-form that opens and fills correctly only in Adobe Acrobat or Reader, and the Central Forms Repository warns that older versions such as Adobe Reader XI no longer work. A French version, 3183F, is posted alongside the English form.
