WSIB Form 10585A: Hearing Aid Replacement or Exception

Hearing health care providers use this four-page form to ask the WSIB to approve replacement hearing aids within the five-year hearing aid cycle, replace lost, stolen or damaged aids, or fund a clinical exception level aid. It records the reason for the request by ear, the current and requested devices, and the clinical rationale. The WSIB must approve an exception-level aid before it is dispensed.

Who completes it

The audiologist or hearing instrument specialist completes the provider sections, and clinical exceptions and prescriptions need an audiologist and/or physician signature in Section E. The patient completes Section D on how they use their hearing aids and signs the Section F acknowledgement in person.

Submitting and payment

How to submit it
Upload the form with the prescription, the Hearing assessment form (3275A) and other supporting documents at wsib.ca/submit, or mail it if you cannot access the website.
Who pays
When the WSIB approves replacement aids for a prescription change, your services are paid through the mini bundle (BSHAMINL/BSHAMINR for audiologists, BSHHMINL/BSHHMINR for hearing instrument specialists), allowed once the mini bundle outcome report is received.

Good to know

  • Replacement within the five-year cycle may be authorized for a hearing change of 20 dB or more at three frequencies between 500 and 3,000 Hz, fitting issues, or changed work duties.
  • Other accepted reasons include a medical change in the ability to operate the aids, bimodal use, and repairs once accumulated manufacturer repairs make the aids no longer viable.
  • Patients should trial an entry to mid-level aid from the other preferred manufacturers before an exception-level request, which needs the manufacturer invoice for the mid-level aid.
  • For lost, stolen or damaged aids, the patient must submit the Declaration of lost, stolen or damaged hearing devices form (10570A), and the WSIB generally considers one such replacement per five-year cycle.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

When will the WSIB replace hearing aids before five years?

When there has been a change in hearing of at least 20 dB at three frequencies in the 500 to 3,000 Hz range, a medical change in the ability to operate the aids, fitting issues, a change in work duties and environment, bimodal use or other medical causes, or repeated manufacturer repairs that make the aids no longer viable.

Who qualifies for a clinical exception hearing aid?

The WSIB's guidelines describe people whose clinical needs are not met by entry or mid-level devices, such as severe to profound hearing loss of 70 dB or poorer, severe to profound single-sided deafness, bimodal users, or combined hearing and vision loss. Physical fit and changed work duties can also qualify. Pre-approval is required before purchase.

What must be attached to Form 10585A?

For a replacement or clinical exception, attach supporting medical documentation, the prescription and the Hearing assessment form (3275A); an exception request also needs the manufacturer invoice for the mid-level aid. For lost, stolen or damaged aids, include the manufacturer's invoice, repair estimate or confirmation the aid is damaged beyond repair. Audiogram reports must be within six months of the test.
