WSIB Mini Bundle Outcome Report (10692A)

When the WSIB approves replacement hearing aids because of a new prescription after a patient's first year of bundled hearing services, this three-page report closes the mini bundle. It records the service and fitting dates, the patient's 15-item outcome questionnaire and the final device. The provider completes it with the patient at the end of the 30 to 90-day trial, once the final aid is chosen.

Who completes it

The hearing health care provider who did the initial assessment completes it with the patient and signs Section F. The patient completes the Section D questionnaire and signs the Section G declaration in person.

Submitting and payment

How to submit it
Log in at wsib.ca/submit and submit it between 30 and 90 days after dispensing the replacement aids, with the manufacturer's invoice for the new aid if not already sent.
Who pays
Bill the report under BSHORMIN; the mini bundle itself (BSHAMINL/BSHAMINR for audiologists, BSHHMINL/BSHHMINR for hearing instrument specialists) is allowed only once this report is received.

Good to know

  • The WSIB must pre-approve the replacement before you submit this form, following its replacement and clinical exception guidelines and the request form 10585A.
  • Lost, stolen or damaged aids are not reported here; the form directs those to the Hearing aid replacement and/or clinical exception request form instead.
  • Use the hearing aid trial start date as the date of service for the mini bundle; a report completed too early is not considered valid.
  • The mini bundle applies only when someone needs a new prescription for a replacement device, and home visits during it are included in the fee rather than billed separately.

When a patient is approved for replacement hearing aids due to a new prescription after completing their first year of bundled hearing services. The WSIB must pre-approve the replacement first. Audiologists bill BSHAMINL or BSHAMINR and hearing instrument specialists BSHHMINL or BSHHMINR, by ear, using the trial start date as the service date. The report itself is billed under BSHORMIN.
