WSIB Hearing Aid Selection Change Form (10688A)

During the 30 to 90-day trial of a patient's initial hearing aid, this two-page form tells the WSIB which aid was finally selected when it differs from the one first dispensed. It records the aid returned to the manufacturer for credit and the new aid chosen, with model, serial number, price and WSIB product code. A new form is completed each time the patient tries a different aid.

Who completes it

The assessing audiologist or hearing instrument specialist completes sections A to E and signs. The patient signs the Section F declaration in person, confirming they received the new hearing aid.

Submitting and payment

How to submit it
Log in at wsib.ca/submit and upload the form with supporting documents during the 30 to 90-day trial period, along with the manufacturer's invoice for the new hearing aid if you have not already sent it.
Who pays
No separate fee is listed for this form, since selecting and dispensing the first hearing aids is covered by the initial bundle, which is paid once the initial bundle outcome report is received.

Good to know

  • You no longer need WSIB prior approval to trial different entry and mid-level aids during the trial period; this form reports the final choice when it differs from what was billed.
  • The trial runs a minimum of 30 and a maximum of 90 days after fitting, and the WSIB says it does not extend beyond 90 days.
  • Clinical exception level aids follow a separate route: the WSIB must approve them before dispensing, using the Hearing aid replacement and/or clinical exception request form (10585A).
  • Section C records the date the returned aids went back to the manufacturer for credit; Section D records the order date and dispense date of the new aids.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

When do I need the WSIB hearing aid selection change form?

When the final hearing aid a patient selects during the 30 to 90-day trial differs from the aid first dispensed and billed. The WSIB's hearing services program asks for the form during the trial period, and for a new form each time the patient tries a different aid.

Does the patient have to sign the selection change form?

Yes, in person. The assessing hearing health care provider completes the form and must have it signed by the patient, who declares in Section F that they received the change in hearing aids described in Section D. The provider signs Section E.

Do I need WSIB approval to switch hearing aids during the trial?

Not for entry and mid-level aids. The WSIB's hearing services program says prior approval is no longer needed to trial different entry and mid-level aids during the trial period, and you report the final selection on this form. Exception-level aids still need WSIB approval before dispensing.
