WSIB Hearing Assistive Technology (HAT) Form 10584A

Audiologists and hearing instrument specialists use this three-page form to get WSIB pre-approval before dispensing hearing assistive technology, such as telephone amplifiers, television devices, alerting systems and FM systems, to a patient with work-related hearing loss. It covers the initial purchase and later repair or replacement. The WSIB's hearing services program asks for it after the hearing aid trial ends and an outcome report confirms the patient is satisfied.

Who completes it

The audiologist or hearing instrument specialist completes the practitioner, patient and request sections and signs the declaration. The patient signs an acknowledgement that their provider is seeking WSIB approval on their behalf.

Submitting and payment

How to submit it
Upload the completed form and supporting documents at wsib.ca/submit, or mail it if you do not have access to the website. Requests for FM systems need an audiologist authorization and a current hearing assessment.
Who pays
Item prices come from the HAT product categories and negotiated manufacturer pricing on the TELUS Health provider portal, and the hearing services fee schedule lists no separate fee for completing the form.

Good to know

  • The WSIB must approve the request before you dispense any hearing assistive technology, and it allows HAT on a case-by-case basis.
  • For lost, stolen or damaged HAT, the patient must first submit the Declaration of lost, stolen or damaged hearing devices form (10570A) for the WSIB to consider replacement.
  • Replacement is considered when a HAT is out of warranty and no longer functional, and the WSIB requires a manufacturer report or other supporting documentation first.
  • FM systems carry a 24-month parts and labour warranty and a 12-month service warranty, according to the WSIB's information about hearing devices.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What counts as hearing assistive technology for the WSIB?

The form describes HAT as items that present auditory, visual or tactile information to people with work-related hearing loss, with telephone amplifiers, television-specific devices, alerting systems and FM systems as examples. The WSIB's hearing device page adds that HATs link the wearer to the sound source and are allowed case by case.

When should I submit the HAT request form?

The WSIB's hearing services program asks providers to submit it after the hearing aid trial period ends and an outcome report confirms the patient is satisfied. The trial runs at least 30 and at most 90 days after fitting. The WSIB must approve the request before you dispense the technology.

What does Section B of the HAT form ask?

Section B covers replacement and lost, stolen or damaged HAT. You indicate whether the WSIB previously replaced a HAT for the patient, whether it is within the manufacturer's warranty, whether the manufacturer tested it and whether it is unrepairable, then list the current model, serial number and original dispense date and attach the manufacturer invoice or repair quote.
