WSIB Ongoing Hearing Services Report (10687A)

After a patient's first year in the WSIB hearing services program, this two-page report documents each ongoing-bundle visit. It confirms the mandatory services were delivered (hearing aid inspection with minor repairs, cleaning and maintenance, and a physical ear check) and lists any others, such as hearing tests or programming. It is completed at the first visit and again at the 180-day mark if the patient returns.

Who completes it

The audiologist or hearing instrument specialist completes sections A to D and signs. The patient signs the Section E declaration in person, confirming they received the services in Section C and are satisfied.

Submitting and payment

How to submit it
Log in at wsib.ca/submit and submit the report with the ongoing bundle bill at the first visit and then at the 180-day mark; visits in between need no form.
Who pays
Bill the report under BSHORONG and the ongoing bundle under BSHCONGL or BSHCONGR for each ear, each billable every 180 days and submitted together.

Good to know

  • The ongoing bundle starts at the patient's first maintenance visit after the first year (initial bundle), and its service date is the day of that first in-person visit.
  • To bill the full annual amount for both ears, the patient must request at least two visits at 180-day intervals.
  • Home visits during the ongoing bundle need WSIB pre-approval and can be billed with approval, unlike the initial and mini bundles, where they are included.
  • If the patient changes clinics within 180 days of the last service date at the original clinic, the new clinic cannot bill the WSIB for that period.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

How often is the WSIB ongoing hearing services report completed?

Only at the first visit of the ongoing bundle and again at the 180-day mark, around six months later, if the patient returns, however many times they visit. The ongoing bundle can be billed twice a year at least 180 days apart, and each bill is submitted with this report.

What services are mandatory at each ongoing hearing aid visit?

Every visit must include a physical inspection of the hearing aid, including the receiver, with minor repairs and parts replacement; cleaning and maintenance of the device; and a physical ear check. The form also lets you record a hearing test, cerumen management, real ear verification, impressions, electroacoustic analysis, programming and manufacturer repairs.

Is the ongoing hearing services report available in French?

Yes. The WSIB publishes a French version, Rapport sur les services auditifs continus (10687B), and the English form notes that it is available in French. The patient must sign either version in person before you submit it through wsib.ca/submit with the ongoing bundle bill.
