WSIB ENT Consultation Report (Form 1787A)

Otolaryngologists use this one-page WSIB report after seeing a patient with hearing loss, tinnitus, dizziness or balance complaints, most often in a noise-induced hearing loss claim. It records occupational and recreational noise exposure, an ear examination, audiogram results and your diagnosis. Related forms are the worker's report on work-related noise-induced hearing loss (0032A) and the hearing assessment (3275A).

Who completes it

The otolaryngologist (ENT specialist) completes the whole form, including the billing section, and signs it or ticks the electronic-signature box and types their name and the date. The worker files their own report on work-related noise-induced hearing loss (0032A) separately.

Submitting and payment

How to submit it
Complete the PDF, save it and upload it with the audiograms and any earlier reports through WSIB online services at wsib.ca/submit, which requires an online services login; fax is also accepted.
Who pays
The WSIB pays the report under service code M647, printed in the form's billing section and listed in the physician fee schedule as a consultation report billed with a consultation visit, and fee code M650 applies to enclosed copies of previous consultation reports or audiograms.

Good to know

  • The occupational noise section asks for type of work, hearing protection, years of exposure, noisy hobbies such as hunting or snowmobiling, and gun use with years, shoulder and rounds per year.
  • Tick whether audiograms with readings at 500, 1000, 2000 and 3000 Hz are enclosed, along with any previous assessments and previous audiograms.
  • The history section has checkboxes for head injuries, family history, allergies and ear surgery, and asks you to list any ototoxic medications.
  • The physician fee schedule says to submit copies of your own medical records only; the WSIB requests reports from other physicians involved in the case itself.

The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.
