WSIB Form CMS8 (0010A): Occupational Mental Stress

This is the WSIB's first report for a work-related mental stress condition, used instead of Form 8 when a patient says their psychological condition is due to work or you believe it is. Only physicians and nurse practitioners can complete it, after the initial visit. It gives the WSIB a DSM-based diagnosis, treatment plan and return-to-work information for adjudication and case management.

Who completes it

A physician or nurse practitioner completes Sections B to F and signs the return-to-work page. The patient completes Section A (patient and employer information) and signs Section G to authorize giving page 3 to their employer.

How to fill it in

A. Patient and employer information (patient to complete section A)

The patient enters their name, address, telephone, date of birth, sex and language, the employer's name and address, a supervisor or contact name with telephone, and their job title or occupation. Add the claim number at the top if one exists. Review the section for completeness before submitting; the WSIB collects this information to administer the Workplace Safety and Insurance Act.

B. General section

Record whether the patient says their psychological condition is due to work, the date they first sought medical care for it and the date symptoms began. Indicate whether the condition continues, or give the date of last symptoms or resolution. Question 3 asks, in your own words, for your understanding of the work-related situation or situations that led to the reported condition.

C. Clinical information section

Document the diagnosis and, if met, the criteria from the Diagnostic and Statistical Manual of Mental Disorders (DSM); the form asks for a DSM diagnosis where possible. WSIB's chronic mental stress policy requires a DSM diagnosis before a claim can be adjudicated. Then answer whether you know of pre-existing or co-existing psychological conditions or other contributing factors (yes, no or unknown), briefly describing the diagnosis, onset date and previous treatment if known.

D. Treatment plan

Describe the treatment plan, including the type of treatment, its duration, prescribed medications and any recommended referrals. Sections B to E sit on page 2, which goes to the WSIB along with page 3. Only page 3 is given to the patient, who can pass it to their employer if needed, so the clinical details on page 2 do not go to the employer.

E. Billing section

Tick physician or nurse practitioner, then enter your WSIB provider ID, invoice number, service date, name, address, telephone and fax. The service code is pre-printed as 8CMS; the physician and nurse practitioner fee schedules list the report under CMS8 as a paper-submission fee. Fill in the HST registration number and amount, under code ONHST, only if HST applies.

F. Return-to-work information: must be completed by a health professional

Page 3 is the only page the patient gives their employer. Say whether the patient lost time from work; if they are off, tick regular or modified duties with a start date and any graduated hours, or that they cannot work, with an explanation and what would need to be in place for a return in any capacity. Describe functional abilities (full, impaired with accommodations required, or other limitations), add the follow-up plan, then sign.

G. Patient's signature

The patient signs to authorize you to give their employer a copy of page 3 outlining their functional abilities, and acknowledges that a copy goes to the WSIB. When the form is completed and submitted electronically, you and the patient can each tick the box that stands in for a signature, but the name and date fields must still be filled in.

Common mistakes

  • Using Form 8 for a work-related mental stress condition; CMS8 is the report for occupational mental stress, and only physicians and nurse practitioners can complete it.
  • Leaving Section C without a diagnosis: WSIB policy requires a DSM diagnosis before a chronic mental stress claim can be adjudicated, and the form asks for DSM criteria if met.
  • Giving the patient pages 2 and 3 for their employer; only page 3, the return-to-work page, is meant to be passed on.
  • Submitting a second CMS8 for the same person or using it as a progress report; the WSIB pays one per person after the initial visit.
  • Completing a Functional Abilities Form at the initial visit, when the WSIB does not pay for an FAF submitted with a CMS8.
  • Ticking the electronic-signature box without entering your name and the date, which the form says must be filled in for the box to count.

Submitting and payment

How to submit it
Send pages 2 and 3 to the WSIB online at wsib.ca/submit or by fax after the initial visit, and give the patient a copy of page 3 only to pass to their employer if needed. WSIB describes prompt submission as critical to processing the claim.
Who pays
The WSIB pays physicians and nurse practitioners a set fee for one CMS8 per person after the initial visit (listed as CMS8 in their fee schedules, with service code 8CMS printed on the form), and does not pay for an FAF completed the same date.

Good to know

  • Tell the patient they must report their mental stress injury to the WSIB at wsib.ca/report, and that their employer must also report it there.
  • WSIB's chronic mental stress policy requires a DSM diagnosis before a claim can be adjudicated; physicians, nurse practitioners, psychologists and psychiatrists can provide that diagnosis.
  • If the patient also has a work-related physical injury or illness, you can complete a Form 8 for it on the same day as the CMS8.
  • The WSIB accepts only its approved CMS8, not a clinic-made version. The current release is 0010A (07/26), and the fillable PDF can be completed and saved.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can complete WSIB Form CMS8?

Only physicians and nurse practitioners. Other health professionals cannot use a CMS8, although WSIB's chronic mental stress policy accepts a DSM diagnosis from psychologists and psychiatrists as well as physicians and nurse practitioners. Chiropractors and physiotherapists report physical work injuries on Form 8 instead.

Can I complete Form 8 and Form CMS8 on the same day?

Yes, when there is a concurrent work-related physical injury or illness. The CMS8 covers the occupational mental stress condition and Form 8 covers the physical injury. What the WSIB will not pay at the initial visit is a Functional Abilities Form submitted with either report; the patient or employer can still request an FAF at a follow-up visit.

How do I submit Form CMS8?

Send pages 2 and 3 to the WSIB online at wsib.ca/submit, where supporting documents can also be uploaded, or by fax. Give the patient page 3 only, the return-to-work page, which they can pass to their employer if needed. Submit one CMS8 per person after the initial visit; it is not a progress report.

Does the WSIB need a DSM diagnosis on Form CMS8?

Section C asks for the diagnosis, a DSM diagnosis if possible, and the DSM criteria if met. WSIB's chronic mental stress policy says there must be a diagnosis in accordance with the DSM before a claim can be adjudicated. In complex cases, the WSIB may require an assessment by a psychiatrist or psychologist.
