WSIB Form REO8 (2234A): Continuity Report

When a patient reports a recurrence or re-injury of a work-related condition, the treating chiropractor, physician, physiotherapist or nurse practitioner documents it for the WSIB on this one-page report. Its 11 questions cover the history of the recurrence, treatment, other providers and new injuries in the interval, current findings and working diagnosis, and task limitations. The worker gives their own account on the Worker's continuity report (2232A).

Who completes it

The treating chiropractor, physician, physiotherapist or registered nurse in the extended class completes the whole form, ticks their designation and signs the declaration. The worker reports the recurrence separately on the Worker's continuity report (2232A).

Good to know

  • If you are asked to complete a Functional Abilities Form at the same time, the form says you do not need to answer questions 9 and 10 on task limitations and their duration.
  • Question 9 covers standing, sitting, lifting, kneeling, climbing, upper extremity use, heavy equipment, environmental conditions, protective equipment, public transit, driving and medication, and asks whether return to work was discussed.
  • The fee schedules describe this report as used in all cases where the worker has identified a recurrence of a previous work-related injury or illness.
  • The form states that section 37 of the Workplace Safety and Insurance Act authorizes you to release this information to the WSIB, and that knowingly making a false statement is an offence.
