WSIB Follow-up Cannabis Assessment Form (0211A)

Once the WSIB has approved medical cannabis for a work-related condition, it reviews coverage regularly, and this form is how it gathers clinical information from the worker's physician or nurse practitioner for that review. The WSIB sends it when needed; it is not posted publicly. The answers help decide whether cannabis remains a necessary, appropriate and sufficient treatment under the WSIB's Cannabis for Medical Purposes policy.

Who completes it

A physician or nurse practitioner completes the whole form, ideally the professional managing the worker's ongoing care. If another professional authorized the cannabis, WSIB policy says the primary treating professional must support its continued use.

Submitting and payment

How to submit it
Complete the copy the WSIB sends you and return it to the WSIB; if you fax a report, the WSIB fee schedule says not to send the original as well.
Who pays
The WSIB pays for completing it under fee code M631, listed on both the physician and the nurse practitioner fee schedules.

Good to know

  • Policy 17-01-10 sets the first review no more than three months after initial entitlement, then reviews at intervals of no more than six months from the previous one.
  • Continued coverage requires that the worker has been clinically reassessed and that evidence shows cannabis continues to be a necessary, appropriate and sufficient treatment.
  • Under the Cannabis Regulations cited in the WSIB policy, the period of use in a medical document cannot exceed one year, separate from the WSIB's own review schedule.
  • The worker must tell the WSIB about any material change in circumstances, and must get WSIB approval before buying medical cannabis or a vapourizer.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.
