WSIB mTBI Care and Outcomes Summary (10393A)

The WSIB requires this four-page report when a worker completes the mild traumatic brain injury program of care or is discharged, including when they stop attending. It records employment status at discharge, whether the person has returned to pre-injury function, outstanding symptoms, response to treatment, the Rivermead score, Patient-Specific Functional Scale scores from mid-point to discharge, and accommodations for return to work.

Who completes it

The registered regulated health professional delivering the program, such as a chiropractor, occupational therapist or physiotherapist, completes and signs the report.

Submitting and payment

How to submit it
Submit it through wsib.ca/submit within two business days of the last treatment session or discharge; the WSIB does not pay the report fee if it has not received the report within 14 days.
Who pays
Bill the report under MTBRCOS, the code printed on the form; block two payment (MTBRTB2 or MTBRTB2V) is withheld until the WSIB receives the corresponding report.

Good to know

  • Employment status at discharge is recorded as full or part time, regular or modified duties, regular or modified hours, or not working.
  • Accommodation tables cover physical limits, mTBI symptom triggers such as computer work, lighting, noise and dealing with the public, and safety issues like work at heights, driving and machinery.
  • Complicating factors to flag include believing hurt equals harm, fear-avoidance, low mood, preferring passive treatments, and home, relationship or work environment concerns.
  • A supplementary block is available for people approved for additional community treatment, and it is reported on Form 10394A.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

When is the WSIB mTBI care and outcome summary due?

The form says you must submit it when the injured person completes the mTBI program of care or whenever they are discharged, including if they did not return or self-discharged. The fee schedule asks for it within two business days of the last treatment, and the fee is not paid if it arrives after 14 days.

Which scores go on the mTBI care and outcome summary?

The Rivermead Post-Concussion Symptoms Questionnaire total out of 64, excluding other difficulties, and the Patient-Specific Functional Scale for three to five activities, at least two work-related, comparing mid-point and discharge scores. You also rate response to treatment, from fully recovered to worsening, and say whether the person has returned to pre-injury function.

Is Form 10393A available in French?

Yes. The French version is the Sommaire des soins et des résultats dans le cadre du Programme de soins pour les lésions cérébrales traumatiques légères (10393B), and the English form notes that it is available in French. Submit either version through wsib.ca/submit.
