WSIB Community Mental Health Progress Form (0203A)

At the end of each Community Mental Health Program treatment block, the psychologist reports progress to the WSIB on this four-page form. A block is six sessions over up to eight weeks. The form records sessions delivered, response to treatment, attainment of up to four SMART goals, an updated DSM diagnosis and functional status, the plan for further treatment, and an opinion on the worker's occupational function.

Who completes it

The program psychologist or psychological associate completes and signs the whole form, either printing and signing it or typing and submitting it electronically. The worker and employer do not complete any part.

Submitting and payment

How to submit it
Submit it online through WSIB online services at wsib.ca/submit, or by fax, at the end of every 6th session or 8th week of treatment, whichever comes first.
Who pays
The form carries its own billing fields: your WSIB provider ID, invoice number and service code MHPBTF, plus HST registration and the ONHST code if HST applies.

Good to know

  • Mark whether the worker completed the block or did not return or self-discharged, and give the block number, sessions provided and treatment period.
  • Goal Attainment Scaling rates up to four SMART goals from much better to much less than expected, with a status such as in progress, completed or revision required.
  • The treatment plan section offers three choices: no further treatment, continue treatment as authorized, or more treatment beyond the program, which requires a call to the WSIB.
  • The WSIB says its mental health program of care, delivered from April 27, 2026, replaces the Community Mental Health Program; confirm which form applies before reporting.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

How often is the WSIB CMHP progress form due?

The program's assessment form says a progress form is required at the end of every 6th session or 8th week, whichever comes first. The progress form itself describes a treatment block as six sessions over up to eight weeks, and asks you to record the block number, sessions provided and the treatment period covered.

What are SMART goals on WSIB Form 0203A?

The form expects the psychologist and patient to set goals that are Specific, Measurable, Achievable, Relevant and Time-bound, aimed at symptom reduction and functional restoration, including return to work. For each of up to four goals, rate attainment against expectations and give its status, such as completed or no further gains anticipated.

Can I ask to speak with WSIB staff on the progress form?

Yes. Section E asks whether you would like a case file discussion with WSIB staff, alongside questions on imminent risk of harm, barriers to returning to occupational function, functional abilities or restrictions with expected duration, and whether the patient would benefit from a Specialty Program assessment or other intervention.
