WSIB Form 8S (0407A): Occupational Chest Disease

The WSIB sends out this physician's report when a patient reports a work-related occupational chest illness; you complete it and return it to the WSIB. Conditions adjudicated in its Occupational Disease and Survivor Benefits Program include pneumoconiosis, such as asbestosis, silicosis and talcosis, and respiratory disorders such as asthma, COPD and hypersensitivity pneumonitis. The WSIB supplies the form rather than posting it for download.

Who completes it

The physician completes the whole form. The worker's claim (Form 6) and the employer's report (Form 7) are filed separately, and the WSIB may ask the worker to sign a permission form so it can obtain a specialist's report.

Submitting and payment

How to submit it
Complete the copy the WSIB supplies and return it to the WSIB; if you fax it, the physician fee schedule says not to also send the original.
Who pays
Bill the WSIB directly under physician fee code M656, the code for the physician's report on occupational chest disease.

Good to know

  • The WSIB's occupational disease list includes respiratory disorders from acute or chronic workplace exposures: asthma, bronchitis, rhinitis, fibrosis, alveolitis, hypersensitivity pneumonitis, emphysema and COPD.
  • Pneumoconiosis claims (asbestosis, silicosis, and talcosis from talc exposure) and all cancers are also adjudicated in the WSIB's Occupational Disease and Survivor Benefits Program.
  • For permanent impairment from work-related asthma, WSIB policy 16-01-01 says the WSIB itself arranges all pulmonary function and histamine or methacholine challenge tests.
  • Other occupational illnesses use different WSIB physician reports: Form 8D (0307A) for occupational disease generally, Form 8D2 (0353A) for skin disease and Form 8W (0425A) for vibration white finger.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Which chest conditions does the WSIB adjudicate as occupational disease?

The WSIB's list for its Occupational Disease and Survivor Benefits Program includes pneumoconiosis (asbestosis, silicosis and talcosis) and respiratory disorders related to acute or chronic workplace exposures: asthma, bronchitis, rhinitis, fibrosis, alveolitis, hypersensitivity pneumonitis, emphysema and COPD. All cancers are adjudicated in the same program.

What billing code applies to WSIB Form 8S?

Physician fee code M656. The WSIB physician fee schedule lists it in the physician reports table and describes it as a form the WSIB sends when a person reports a work-related occupational chest illness, to be completed and returned to the WSIB. Bill the WSIB directly; accounts received more than six months after the service may be reduced.

Is there a time limit for an occupational chest disease claim?

The WSIB states there is no time limit for submitting an occupational disease claim. The patient can file through their employer, doctor, union or a representative, or on their own, and the WSIB asks them to describe their symptoms, when they started, the substances they were exposed to and their work, and to list all doctors seen.
