WSIB Physiotherapy Assessment Report (0856A)

Physiotherapists complete this one-page WSIB report when physiotherapy treatment begins for a work-related injury. Its nine questions cover the referring health professional, injury history, physical findings, working diagnosis, treatment goals and proposed program, whether the patient can work during treatment, physical restrictions, expected recovery and factors that may delay it. The WSIB pays one report per injury or recurrence.

Who completes it

The treating physiotherapist completes and signs the whole form, including the patient and employer details and the billing block.

Good to know

  • The WSIB forms page lists this report as 0856C, while the PDF itself is numbered 0856A (04/12); both refer to the same physiotherapy assessment report.
  • Since May 1, 2023, community-based musculoskeletal treatment must be delivered through the WSIB MSK program of care, which uses its own initial assessment report (10635A).
  • Only one report is paid per injury or recurrence, so complete it once, when physiotherapy treatment begins.
  • Question 9 asks for factors that may delay recovery, including pre-existing or underlying conditions, and question 6 asks whether the patient can work while in treatment.
