WSIB IDT Program of Care Reports (10708A to 10715A)

The WSIB's interdisciplinary team program of care, an eight-week contracted program for complex musculoskeletal injuries, uses four reports, each with a work hardening version. The initial assessment opens the program, the mid-point report closes block one, the care and outcomes summary follows completion or discharge, and a supplementary report applies only when the WSIB approves additional treatment. All track the same outcome measures and abilities for return-to-work planning.

Who completes it

The contracted interdisciplinary team, at minimum a physiotherapist or chiropractor plus an occupational therapist, completes every report, with signatures from the team lead regulated health professional and another regulated team member.

Submitting and payment

How to submit it
Submit each report and supporting documents through WSIB online services at wsib.ca/submit: the mid-point report at the end of block one, and the care and outcomes summary when the person completes the program or is discharged.
Who pays
Payment follows the contract each provider holds with the WSIB after its open procurement process, and the WSIB's list of practitioner fee schedules has no separate schedule for this program.

Good to know

  • Work hardening reports (10709A, 10711A, 10713A and 10715A) are used only when the WSIB has approved work hardening, which has its own reporting requirements.
  • The care and outcomes summary compares initial and current range of motion, strength and outcome scores, and records whether the person completed the program or self-discharged.
  • To recommend a supplementary block, tick the option on the care and outcomes summary and call the WSIB for pre-approval, giving frequency, duration and a requested start date.
  • Each report asks whether the team communicated with other treating professionals, such as the musculoskeletal program of care provider, an orthopedic surgeon or the family physician.

Frequently asked questions

Which reports are in the WSIB interdisciplinary team program of care?

Four standard reports: the initial assessment report (10708A), mid-point report (10710A), care and outcomes summary (10712A) and supplementary report (10714A). Each has a work hardening version: 10709A, 10711A, 10713A and 10715A. The supplementary reports apply only if the WSIB approves additional treatment.

When is the IDT mid-point report due?

At the end of block one. The program runs eight weeks in two four-week blocks, and the mid-point report records the number of sessions in block one, the interventions delivered, the overall response to treatment, recovery barriers and changes in functional abilities since the initial assessment.

Which outcome measures do the IDT reports use?

At least one functional measure related to the injured area, repeated throughout treatment: the Neck Disability Index, Oswestry (ODI 2.1a), QuickDASH with its work module, Lower Extremity Functional Scale or WHODAS 2.0. Where clinically indicated, add GAD-7, PHQ-9 or the Pain Self-Efficacy Questionnaire.
