WSIB Chiropractor Treatment Extension Request (0148A)

Ontario chiropractors use this one-page request when a worker needs chiropractic treatment beyond the initial allowed period. The form warns that the WSIB will not pay for treatment past 12 weeks unless it pre-authorizes an extension. It covers the working diagnosis and any change, treatment results to date, work status, current findings and limitations, expected improvement, and the duration and frequency of further treatment.

Who completes it

The treating chiropractor completes the entire form, including the patient's details and claim number, and signs it, or ticks the electronic-signature box after entering their name and date. The worker and employer do not complete any part.

Submitting and payment

How to submit it
Submit it online through WSIB online services at wsib.ca/submit, or by fax, at least four weeks before the end of the 12-week treatment period so treatment can continue without a gap.
Who pays
The WSIB chiropractic fee schedule states that no fee is paid for completing the Chiropractor's Treatment Extension Request form.

Good to know

  • Do not submit a treatment extension request for anyone being treated in a WSIB program of care; the WSIB says to read the program's FAQs instead.
  • The form cites section 37 of the Workplace Safety and Insurance Act, which authorizes the chiropractor to release the requested clinical information to the WSIB.
  • It asks whether the worker would benefit from a multidisciplinary health care assessment, and for factors delaying recovery and current functional limitations.
  • The PDF's heading reads Chiropractor's treatment extension report. A French version (0148B) is available, and dates are written day, month (three letters) and year.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Does the WSIB pay chiropractors for completing Form 0148A?

No. The WSIB chiropractic fee schedule states that no fee is paid for completing the Chiropractor's Treatment Extension Request form.
